Things to bring to your initial assessment

  • completed questionnaire form ( required PRIOR to signing)
  • birth certificate and / or driver’s license
  • your last pay stub
  • copies of loan agreements and security documents (if available)
  • all credit cards
  • all tax information
  • $200 cash deposit (required if / when signing bankruptcy documents)

Booking an appointment and completing the questionnaire in no way obligates you to make an Assignment in Bankruptcy.  The information requested will assist us in providing you with information on all the options available to you, given your individual circumstances, at your initial assessment.
